Job Description
The RAI-Minimum Data Set (RAI-MDS) Resident Assessment Coordinator (RAC) in the Geriatric and Extended Care Service is a self-directed, quality specialist, professional nurse with a broad-based clinical knowledge appointed to effectively implement health promotion, prevention, and management of skilled care and chronic illness, and care of the dying. The RAI-MDS RAC provides oversight in the coordination and facilitation of the RAI-MDS process/program.

The RAI-MDS RAC has a specialized knowledge of current long term care standards, the Center of Medicare and Medicaid Services and its application to the RAI-MDS process. Duties include but are not limited to:
  • Manage and coordinate the facility RAI-MDS program
  • Accountable for the overall RAI-MDS program process
  • Accountable for changes that effectively support the program and is responsive to the quality measures and other performance improvement measures
  • Provide expertise and knowledge in promoting the application of the RAI MDS assessment and the care planning process, considering the age specific needs of the residents
  • Guide and support the interdisciplinary treatment team in performing the inter-rater reliability studies to assess the accuracy of the RAI-MDS process
  • Analyze the program and identify best practices as related to care, quality and cost associated with the RAI-MDS process including budgetary, staffing, and equipment availability
  • Serve as a resource and expert for RAI-MDS issues to improve process

Pay: Competitive salary, regular salary increases, potential for performance awards
