Job Description
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Qualification
The Patient Aligned Care Team (PACT) Nurse Care Coordinator is a self-directed professional nurse who assumes leadership role in managing a group of patients, coordinating care with physicians, nurse practitioners, medical residents/students, other health professional and hospital systems. The RN assumes leadership in coordinating responsibility for patients assigned to the Patient Aligned Care Team. The RN executes position responsibilities that demonstrate leadership, experience and creative approaches to the management of complex needs of patients. The RN coordinates care for the patient across and between the health care system including inpatient hospitalization and care in the community. The RN uses motivational interviewing skills to guide the patient in establishing goals through patients self-management, supporting prevention and wellness.
Certification in Ambulatory Care or Case Management is expected to be achieved within two years after appointment.
